Physician House Calls
Jewish Senior Life is committed to providing care for older adults throughout our community. For many, leaving their home to receive medical care can be very difficult, whether it's due to immobility, disease, age or injury. Homebound patients are often unable to receive the care they need and may experience feelings of isolation and loneliness. A visit from a kind and compassionate doctor or nurse practitioner can help many individuals and their caregivers overcome these feelings, while addressing their medical needs.
The Jewish Senior Life Physician House Calls Program offers patients:
- In-home primary care and assessments of medical problems
- In-home acute appointments for urgent, non-emergency care for patients of the practice
- Coordination of care with other home health care services
- One-on-one visits with the physician
- Review of medications and methods of taking them
- Assessments of abilities to perform daily activities, such as dressing, bathing, walking and eating
- Assistance in developing a plan of care in the event that the patient’s health worsens
Our Physician House Calls team consists of experts in Geriatrics and Palliative Care, who are committed to providing patient-focused care, and can manage the complex needs of the aging, right in the comfort of their own home. Dr. Brian Heppard, board certified Geriatrician, established and directs the House Calls program. Dr. Heppard is also board certified in Hospice and Palliative Medicine, and is the Medical Director for Jewish Senior Life.
